Beverly P. Horowitz, PhD

beverly Dr. Beverly P. Horowitz is a NYS licensed occupational therapist and social worker, doctorally trained gerontologist, and professional Geriatric Care Manager who provides individualized, compassionate care management services to older adults and their families. She is a Fellow of the American Occupational Therapy Association and a Fellow of the Gerontological Society of America.

Dr. Horowitz has 30 years of experience working with older adults, families, and caregivers in their homes, assisted living facilities, rehabilitation centers, and in nursing homes. She provides client-centered, individualized care to older adults and families to support quality of life for persons of diverse backgrounds experiencing chronic illness, frailty, and/or disability. Dr. Horowitz has expertise in geriatric care management, geriatric rehabilitation, home safety, wellness programs, and client, family, caregiver and professional education and training to help older adults maximize their abilities, cope with declining health, and live with dignity wherever they reside. Client-centered care and partnering with clients and families to support each client’s unique care needs is a hallmark of Dr. Horowitz’s geriatric care management practice.

Dr. Horowitz is Associate Professor in the Occupational Therapy Program at York College-CUNY and has been a core faculty member with the Long Island Geriatric Education Center Consortium at Stony Brook University.
