Beverly P. Horowitz, PhD
Dr. Beverly P. Horowitz is a NYS licensed occupational therapist and social worker, doctorally
trained gerontologist, and professional Geriatric Care Manager who provides individualized,
compassionate care management services. She is a Fellow of the American Occupational Therapy Association and
a Fellow of the Gerontological Society of America.
Dr. Horowitz has more than 30 years of experience working with older adults, families, and caregivers in their homes, assisted living facilities, rehabilitation centers, and in nursing homes. She provides client-centered, individualized care to support quality of life for persons of diverse backgrounds experiencing chronic illness, frailty, and/or disability. Dr. Horowitz has expertise in geriatric care management, geriatric rehabilitation, home safety and environmental assessment and home modifications, wellness programs, and client, family, caregiver and professional education and training to help older adults maximize their abilities, cope with declining health, and live with dignity.
